I have to replace 2 sets of drawer slide in a pair of 48" wide drawers that have about 75lbs of cookware in each one. The existing slides are unbranded ball bearing slides integrated in metal drawer sides with a 1/2" drawer bottom. The existing ball bearing cages are torn apart and ball bearings are falling out.

Any ideas on which slides to use?

Get KV heavy duty side mounts. Trying to fit under mounts is probably not worth the effort.

Pictures of the drawer boxes top, bottom, side would help with suggestions.

I have to make them, the other slides were part of the metal drawer box side

Standard bottom mount Blum on 30 inch drawers all the time.

I've never done them on a 48.... go with what Tom said.

Face frame or frameless?
If it was a face frame I would also pad the sides with vertical blocking to help carry the weight.

If you're using 21" drawers, B569H5330B is the slide number you need.

BT51.1901 are the locking devices you need, add an L or R to get the lefts and rights.

These are 48" drawers. I see I have to adjust the right side before the Parade of Homes.

Theres about 100 pounds of pots and pans in each of these.
