What is the standard way of installing spindles? Do people build the complete section on the ground, and then install, similar to platform framing? Or do they install the 2x4s, then install the spindles?

The 2x4's can be ran inbetween posts or on the outside of the posts. I always screw the spindles on after the 2x is up so their plumb.
I'll pre drill the screw holes and start the screws on the ground, then use a 3 1/2 or 4" block to space the top of the spindles to screw to the railing. Same thing on the bottom but I'll use a level very often to make sure they are plumb as spindles are some times thicker at one end or bowed.

It's best to 'crown' your spindles, - - either way, - - all toward the deck, - - or all toward the yard, - - as long as they're not crowned to either side.

I do it both ways, on the ground/deck if I set the poles (cause I know the're plumb), In the air if it's a retrofit to existing.

Just seems easier.
