Possibly mixed incorrectly, didn't cure correctly, water issue?
Lot of snow last winter. Customer might have hit it pretty hard. Good thought
Im guessing ice melters. See it often.

Yep, I agree with everybody here and say the problem comes from salt or some other de-icing materials.
this pring being key word is either salt as mentioned or he did it late fall and it froze ..more than likely a combination of both
