I got mine from the book Markup and Profit by Michale Stone. It will give you some information for the bsuienss as well as some other information.

Look at the file swap sticky, there is a pretty good boiler plate handbook that is easy to customize.

Mike Finley posted that. It was a form generator and was excellent. It's unfortunately taken off the site now, but I did leave a .doc file posted on the last entry of the thread with what I did.

It's extensive, like 20+ pages or something.
