You could size the "wraps" accordingly to the vents. The larger return could be thicker than the smaller register. Help with the proportions a bit maybe.

I think I'm gonna try clan's way. The smaller grill isn't under a window, it's under the a/c closet. The only problem with framing it is it's the first thing you see when you walk in the front door. I guess if it looks good, it's not gonna be a problem. We'll see. I'll post photos!

I had to shorten that A/C door to 67". See how the grill under the A/C door isn't centered? That makes me think framing it will draw unnecessary attention to it. The big grill I think will look good done that way.

I am sure that took some work but Im not sure why you wouldnt leave room base the baseboard.

Do the door and vent as an entire unit. Lose that bottom lip, and run the same profile as the door underneath it, and blend the vent into it. It will have the look like the vent is just inset in a door. Especially with the fullsize closet door next to it.

You did a nice job with the situation.

When the budget allows, it is nice to literally make the registers part of the woodwork, rather than just running the trim around the metal:
