Just heard a story from on Friday pipe supplier about a new guy they had loading up a bunch of 12" ADS pipe on flatbed and getting on highway without strapping it down. Pipe didn't make it far and he was fired that day. Amazed at how dumb people are sometimes

Our old plumbing sub had a new guy take a fibreglass tub surround in the back of a company truck to one of our sites. The guy didn't strap it down, took a corner too fast and out popped the surround.

I worked in the Sears hardware department during high school. One of my idiot coworkers sold a guy a fiberglass extension ladder and gave him some clip on suction cups to get it home. Problem was that those are meant for aluminum ladders only! They wouldn't hold the heavier fiberglass ladders, and the clips don't fit right on the thicker fiberglass. The ladder ended up on the freeway, damaging the owners car, as well as several others.
