Backcharge form?

I'm a sub-contrator erecting two shop/bay warehouses for a GC and we recently had to set grade on the anchor bolts and modify our center columns. I was wondering if any one has a simple backcharge form so I can charge these guy what they owe us? I lost my template we hadbeen using. I'd appreciate the help.

Sounds like what you should have used was a change-order...and you should have used it before you did the work. Maybe I'm not understanding.

Back Charge form

Did you try one of the Ironworker sites. They might have a simple back charge form. I have some friends who do ironwork. I will ask them.
