Can Anyone Provide Me With A Decent Proposal Form?

I'm looking for a good proposal form that I can scan into my pc using PAPERPORT or similar software. I want to be able to open it and fill in the fields as well as pre-fill boilerplate items.

Please forgive my posting this as I'm sure this has been discussed before. It can be very time consuming looking thru the many many posts using the search feature.

Any help would be very appreciated...
