Tile Repair

so all the tiles i epoxied on 2 years ago fell off along with a few more.... i started prying around and got some more suspect tiles off too, the morter behind them falls out like sand, it is 30 years old, so i guess it's just shot! ended up pulling off about 10 sq. ft. so i would like to put these back on the right way, but not sure how to mix... i am going to grind back the old morter and stick the new ones to some fresh mud. i was thinking white portland and white sand 1:1, and a little lime, is this right? and what ratio should i use for the grout? greatly appreciate any help here.
