Testimonial form

Hey guys,
I have searched here but not much luck finding what I want. I am looking for a form that is used to give clients for testimonies. A form that has a small questionnaire and an option to give their opinion whether good or bad so I can use it as a reference and an option to put it online for marketing. Does anyone have something to share?
