Proposal
A little something I made way back when... It is a copy of the old blank forms we used to get from the local office supply. Its in RTF format so it should work for Apple and PC users.

Just a note, if RTF does not work for you just let me know I can post it in a differnt format, just let me know.

If you have not noticed I broke it up into text blocks so if you watned to you can easly modify this to fit your needs.
