Does anyone have a Notice of Intent to File a Lien for Kentucky?

All Legal Forms are copyrighted. It is illegal to copy and share them. In fact it is illegal to copy one form for continued use. Since you are presenting the form to a court, you might want to simply buy it.

You can find them on line here: http://www.uslegalforms.com/ or simply google it along with the state name.

you dont present a notice of intent to a court. just to the property owner.
