Looking For A Safety Violation Form

About 95% done with my safety manual, but I still need a good recording instrument for a safety violation for writing up an employee.

do you have an office max of office depot? they have employee discipline forms in a tablet of paper. they should be able to work for writting up a saftey violation. this is what im talking about
