I have been using a tweaked version of this form for years- we must have "borrowed" it from the same place.
I also include a copy of it- with "sample" across it- in every contract package.

Yep, straight out of Gary Ransone's book, The Contractor's Legal Kit. A must read for anyone who need help with contracts.

Thanks, precision. My mind is in the gutter as I am being forced to go on vacation tomorrow to someplace warm. When I get back, I'll find out how to attach files the right way.

Note to file: If you are working on imp[roving your contracts, get the above referenced book, The Contractor's Legal Kit. About $90 and worth every dollar. All forms comne on a disc and you can modify them anyway you want.

Here's a generic order form we've used in the past. It auto calculates the line items based on price and quantity, adds tax and gives you the grand total. Might be helpful to some. Just delete the logos and add your own.

The forms that we offer are the result of over 60 combined years in the construction business, you know, the "school of hard knocks" all of us in the construction business attend daily, and hundreds of hours with construction attorneys as well as typesetters, software experts... etc. For someone to cutely say they "borrowed", wink, wink, this, cheapens what we do, cheapens what you do, cheapens this forum and indeed, cheapens the entire construction industry.

Shame on you UNHIQUE!

stealing the form to use and the feel free to use it was not appropriate at all. We can view the form at www.actcontractorsforms.com? If so direct us to the page or Please go ahead and paste up the image, we may want to buy this form.
