Here are some of the questions I've come across-

Do I need to register a buisness name? Just gonna be a self employed contractor

Under buisness entity. What do I check?

Do I claim 100% of new buisness?

Under criminal record...Do I list my one speeding ticket which I paid for and didnt go to traffic school.

Y or no to supervision of control?

What is the 32 hour a week clause?

How do I prove I have $2500 operating capital.

What is the 32 hour a week clause? Question #14 relates to Responsible Managing Employees and the amount of hours they must work per hour. As a sole owner, this question does not apply to you and you can leave it blank/unanswered.

How do I prove I have $2500 operating capital. The CSLB does not ask you to prove this. Just check the box Yes.
