Just a non-electrician's partial guess here, but...has to be for 220 right? Two hots, nuetral (which I know really isn't a nuetral) and a ground?

Here you go

Back in the early days of electricity, there were two different kinds of 120VAC plugs, parallel and tandem. And this outlet was made to support both. At one time, there were two styles of 120VAC/two-pin plugs. One was the regular parallel blade plug and another called "tandem." The outlet was designed so you could use either type. Later outlets would use T-shaped holes for the same purpose.
