No control joints is usually the cause,or they're improperly spaced.Possibly no expansion joint where it meets an adjoining slab.Improper compaction under footing or improper footing.Weak concrete mix.Earthquakes, car wrecks or sometimes they just crack no matter what you do.

Provided that the slab is OK, it is almost certainly the control joint layout.

Does the crack follow the mortar joint?

Is the crack in the middle of a wall or at a corner?

Does the crack point towards an outside corner?

Is the crack over a door or window?

foundation failure due to expansive clay soil shows up as a crack that gets wider the higher up the wall it goes. Can follow the head joints, or even break the unit.
