Any local glass shop will be able to order you a replacement insulated unit,just remember to measure the width first,then height,and the overall thickness of the unit. also,it is important to give them the info about the unit make up,such as thickness of both lites of glass,spacer color and thickness,and if there are any interior muntins or grids inside the unit,industry standards are written as number of horizontal muntins or grid bars as verticles and horizontal ie, 4v and 2h,this tells then that there are 4 verticle and 2 horizontal muntins. but the most important thing is that glazing safety codes require that safety glass be used or installed where existing was,is required and NON safety glass can not be grandfathered in to replace where safety was/is. another thing to consider was the original unit made up with tinted glass,which is always the outboard lite,low-e glass which is always the inboard lite,and if there was any inert gass infill,such as argon gas.
your size of 28 by 76 sounds very close if not exact for a patio door glass size,usually 5/8 inch thickness,this can however vary up to 1 inch or more,past expierence places the cost around $200 for safety glass unit and $135 non safety,if you need any more info,just ask,hope this helps.
