When I was 18 I got a job working at the Hartz pet supply warehouse here in town, another guy and I were building shelves like those. I had about 20 of the side pieces of the shelves fall onto my back while I was crawling under them to cut a band off of a stack that was leaning against a wall. It broke 3 of my ribs, and the only way I could get out was to push with my arms and slide out, which slit my back open. It hurt like hell, but I hadn't been able to draw a breath for a few minutes and I was afraid the other guy would stand there and let me die before he realized he should run for help. Once I was free from the mess the other guy helped me to the break room, the boss came in and told me if I didn't want to work I didn't need to fake and injury. Finally his secretary realized I was really hurt and drove me to the hospital, she owned a 4 wheel drive, not the preferred suspension when you have broke ribs.

Overloaded pallet racking / under spec pallet racking

The owners are lucky how that fell straight down, no doubt if it had fallen over into the next asile it would have been like a deadly game of dominos.
