How Do I Create A Form On My Site?


I need to creat a form on my web site with a couple of questions that gets e-mail to me when submitted. How do i go about doing this?

Really nice way to go is acrobat forms. It comes with Acrobat professional, and it's pretty easy to use, and with one click, people can email in your form directly to you. I haven't looked too closely, but I think there are other pdf makers that do the same for cheaper, like cutepdf.

Very good and reliable. Check it out it is simple to setup. Now I use Front Page and there are tools in it that will create forms.

I'd be happy to help you setup a basic form. However, the response-o-matic.com tool may be exactly what you are looking for in a webform.
