Question

Where Do You Get A Sworn Statement?

I was reading through the books Im supposed to study to get my builders license and in the law booklet it talks alot about all these sworn startments and other documents your supposed to give to the home owner. I was just wondering where do you get these at? Is there some sort of computer program that has all this on it?
