Thats why I use those plastic anchor bolt standoffs and place the critical ones myself. Also make the foundation crew use them for all the regular anchor bolts. Also per code here you are not supposed to wet set the bolts, they should be held with those plastic anchor bolt holders and then the concrete is poured. Some inspectors here won't sign off on the foundation if they don't see those bolts hanging before the pour.

Out here in the land of shaky earth hold downs are old hat.

The framer usually always makes an appearance for a day prior to the pour to lay out and place the hold down anchors. Or forever hold your tongue.

Those relatively new phd's with the lags are a breeze compared to the older ones with the through bolts.

I run into these quite often in my commercial jobs.. ours are usually on the corners and 8' in from the corners. Ours are the BIG ones,,, 7/8 bolts that go all the war to the footing so placement is usually a bit off. We usually lay the quad that it bolts to out using an actual bracket and just squaring off of it. It seems to go quicker if you pilot the screw holes ... We have a little tool kit just for these with a big wrench, big socket .. right angle drill adapters .. pilot bits ect....
