Maybe you didnt have the thinset spread evenly over the area You may have to remove tiles that are affected and redo them.

What type of subfloor was it?
What kind of trowel did you use?

A number of things could of caused the tile to not stick: when you set the tile it did not get full contact with the thin set ( not enough mud under the tile, or you did not press it down hard enough to get it to stick), someone may have stepped on the tile after it was set but before it was dry I could go on and on but this seams to be the leading cause of lose tile.

Yes you are going to have to remove the tile and reset it. Clean the tile if you save it and also clean the thin set out and start over on the tile that are bad and then regrout.

Sorry but I would say all of us have been there done that.

Good luck

Rich

^^ like they said. if its hollow, you dont have a solid bond. the tile is large 12x12 its moving under weight when its stepped on or near. the movement is cracking the grout. you use a trowel or spotted it?

remove and reset it.
